Background. The presence of extensive bone defects significantly complicates the possibility of stable fixation and correct spatial positioning of the revision endoprosthesis components. The primary task of revision surgery is to restore the lost bone. The use of modern implants makes it possible to replace large metaphyseal defects of the tibia and femur, including AORI type 2B ΠΈ 3 bone defects.
The aim of the study was to evaluate the long-term results of surgical treatment of patients with extensive bone defects (2B and 3 according to AORI) of the proximal tibia and/or distal femur using custom-made implants for revision knee arthroplasty.
Methods. A retrospective clinical study was performed in 24 patients who underwent revision arthroplasty using individual implants (9 femoral and 18 tibial) made on a 3D printer in the period from 2017 to 2021. T3 defect according to AORI classification was diagnosed in 12 patients (50%), F3 defect in 1 (4.17%), F2B in 8 (33.3%), T2B in 6 (25%). All patients before surgery and 3, 6, 12 months after surgery were surveyed according to the international scales VAS, KSS, WOMAC and SF-36.
Results. At the follow-up examination 12 months after the surgery, 9 out of 24 patients (37.510%) walked without additional means of support, 10 out of 24 (41.710%) with a cane, 4 out of 24 (16,78%), walkers 1 out of 24 (4.210%). There were no periprosthetic fractures during surgery and in the postoperative period. When evaluating the functional state of the knee joint, an excellent result was registered in 45.810% and 33.310%; good in 37.510% and 54.210%; satisfactory in 16.678% and 12.57% according to the KSS and WOMAC scales, respectively. When assessed on the VAS scale, there was a positive trend in the form of statistically significant decrease in pain in all patients (p0.01).
Conclusion. The use of individual implants made using additive 3D printing technologies in revision knee arthroplasty in the presence of extensive bone defects (2B and 3 according to AORI) allows to perform an organ-preserving surgery without loss of the statodynamic function of the lower limb. This study has limitations due to the small sample size. We believe that it is promising to study the long-term results of surgical treatment of patients with extensive bone defects (2B and 3 AORI) of the proximal tibia and /or distal femur using individually manufactured implants for revision knee replacement and a comparative analysis of these results with those using conventional implants.ΠΠΊΡΡΠ°Π»ΡΠ½ΠΎΡΡΡ. The aim: to improve the results of treatment of degenerative hipβs diseases in children by means of pathogenetic pattern. Background. Hip arthroscopy is a minimally invasive surgical technique most commonly performed to correct femoroacetabular impingement (FAI). Currently, it is widely used by worldwide; in addition, there are a significant number of english-language publications on this topic. The number of hip arthroscopy performed in Russia is unknown. Information about this surgical technique is also very limited in our country.
The aim of the study was to assess the level of hip arthroscopy in Russia.
Methods. A total sociological survey of orthopaedic surgeon with experience in performing hip arthroscopy was conducted. The questionnaire was posted on the Google Forms platform and consisted of 13 questions of closed, semi-closed and open types. The survey included 54 surgeons from Russian clinics, 45 people filled out the questionnaire.
Results. Among the respondents, doctors with experience in performing arthroscopic operations on the knee and shoulder joints predominated (100.000.00% and 93.300.03%, respectively). The majority of specialists (73.300.06%) were trained in hip arthroscopy during the primary course in one of the clinics in Russia. More than half of the respondents (68.800.06%) are currently engaged in this surgical area, however, in 45.160.07% of them, the number of operations does not exceed 5 per year. For surgeons who have completed two or more training courses, the volume of operations performed is higher (p0.05); 51.110.07% of doctors perform arthroscopy using an alternative technique. There are 2.5 time more specialists doing just bone resection, than surgeons who apply any kind of reconstruction technique while treating FAI (p0.05). Fifteen respondents (48.390.08%) perform debridement as an attempt to delay arthroplasty. The most common difficulties faced by surgeon are problems with the diagnosis of FAI (no patients) (40.000.02%), lack of the necessary tools (40.000.02%) and sufficient time to master the technique (33.300.07%). Only three (6.600.07%) respondents believe that they manage to achieve the planned results of the operation, 93.300.03% of surgeons said that it is not always possible to achieve the desired outcomes.
Conclusion. Hip arthroscopy in Russia is not very common, the volume of such interventions is insignificant. Doctors with minimal skills in arthroscopic hip surgery predominate. Factors that impede the development of this area in our country are related to teaching methods, problems with FAI diagnostics, the lack of necessary instruments for performing operations, and the lack of time for specialists to master surgical techniques.ΠΠΊΡΡΠ°Π»ΡΠ½ΠΎΡΡΡ. Background.The cultured cheek mucosa epithelium (buccal epithelium, BE) is used for autologous transplants generation and tissue engineering. An alternative source of cells for these purposes may be the lip mucosa, covered, like BE, with non-keratinized stratified squamous epithelium, but with some histological distinctions.Aims to characterize the human lip mucosa as a promising source of epithelial cells for autologous transplantation and tissue engineering.Methods.Scrapings of the lip, cheek, and gum mucosa from five healthy volunteers were analyzed by cytofluorimetry to determine the level of desquamation and cytokeratin (CK) 10 and 13 expression. The lip mucosa of two patients was characterized using routine histological staining and fluorescence immunohistochemistry for CK 3, 4, 10, 13, and p63 marker. 35 samples of full-thickness strips of the patients lip mucosa were used to set the explant (n=18) and enzymatic (n=17) techniques for expansion epithelium. Culture systems with 1.05 and 0.06mM Calcium contained 5% fetal bovine serum, 5 g/ml human insulin, 5 g/ml hydrocortisone, 10 ng/ml human epidermal growth factor. Stable cultures were stained for p63, vimentin, zonula occludens-1 (ZO-1), and CK10. Software tools determined levels of their expression.Results.The number of cells in the lip and gum samples was significantly lower than from the cheek. The median number of CK13 positive cells was significantly different for the gum (6.4%) and cheek (64.8%, p=0.0089). Significant differences for CK10 positive cells were not observed. The epithelium of the lip mucosa was 72.13.6 m thick, relatively flat, and without keratinization sites. Samples were positively stained for CK 4 and 13, in the absence of expression of CK 3 and 10. The primary culture of epithelial cells obtained by explant technique was significantly more effective (p=0.001) in comparison with the enzymatic method. Stable cultures had a cobble-stone morphology in both culture systems. The levels of vimentin and p63 expression in both culture systems was not significantly differ. ZO-1 expression was 3.6-fold higher for 1.05-mMCa++medium (p=0.0006).Conclusions.Epithelium cell culture from the lip mucosa can be obtained by culturing explants without a feeder layer. Employment of custom triflange acetabular components (CTAC) is one of the few options for pelvic reconstruction in the patients requiring complex acetabulum revisions with Paprosky 2 C, 3 A and 3 B defects and pelvic ring disruption.The purpose of the study was to describe the features of planning, surgical technique, and short-term treatment outcomes of the patients with significant acetabular defects, in which the revision hip arthroplasty was performed using the CTAC.Materials and Methods. A single-center analysis of a series of consecutive patients was performed: 50 complex acetabulum revisions in 47 patients (16 men and 31 women). The average age of the patients was 60Β±12 years (from 31 to 82; Me 62 years), the average body mass index was 29.7Β±6.3 kg/m2 (18.4 to 46.3; Me 29.0 kg/m).Results. The mean follow-up was 22Β±13 months (from 3 to 3.6 years; Me 20 months). The average Harris score increased from 27Β±7 (from 15 to 39; Me 27) before surgery to 64Β±16 (from 22 to 90; Me 67) a year after (p0.001). The level of pain according to VAS before surgery was 7Β±1 points (from 5 to 9; Me 7), after treatment it decreased to 2Β±1 points (from 0 to 7; Me 1, p0.001). In 17 cases out of 50 (34%), there was at least one complication: dislocations β 7 (14%) cases; infection β 3 (6%); loosening β 2 (4%); complications associated with the femoral component β 4 (8%), including 3 intraoperative fractures and 1 postoperative, associated with loosening of the implant; pronounced disturbances of static and locomotor functions β 2 (4%). 12 cases out of 50 (24%) required another surgery, all of which were performed in a year. The Kaplan-Meyer survival rate for the hip implants was 0.71, for the CTAC β 0.87.Conclusion. Employment of the CTAC for revision hip arthroplasty in the patients with significant acetabulum defects and pelvic ring disruption allows reliable fixation of the endoprosthesis components. The STAC placement technique is more anatomical than use of structural allografts, several augments or sup-cage systems. It allows reconstruction of extensive bone defects, theoretically avoiding the long-term problems with allografts, modular trabecular components, antiprotrusion systems and cup-cage. Brane Decay and Death of Open Strings
We show how open strings cease to propagate when unstable D-branes decay. The
information on the propagation is encoded in BSFT two-point functions for
arbitrary profiles of open string excitations. We evaluate them in tachyon
condensation backgrounds corresponding to (i) static spatial tachyon kink (=
lower dimensional BPS D-brane) and (ii) homogeneous rolling tachyon. For (i)
the propagation is restricted to the directions along the tachyon kink, while
for (ii) all the open string excitations cease to propagate at late time and
are subject to a collapsed light cone characterized by Carrollian contraction
